New Client Paperwork

We strive to be a paperless office. Once you have scheduled your intake session with one of our counselors, you will be emailed a link to our confidential client portal to complete your paperwork online. If you prefer to print and hand write the forms, let your counselor know when you schedule the intake and hard copy versions will be provided to you.

We applaud you for taking the next step! We offer free 15-minute phone consultations if you’d like to talk about your specific situation before scheduling an intake. Feel free to leave a confidential voicemail for any of our counselors on their direct number or you may contact Prosper Counseling's office at
972-292-0007 to discuss the best counseling fit for your needs.
We strive to return all calls within 24 to 48 business hours (M-F). You can also send an email to ProsperCounseling@gmail.com or complete the "Contact Me Form" below and someone will reach out to you.

The first appointment is the Intake Session where we will review your paperwork, talk about your specific concerns, and discuss a plan to move forward. If counseling is for your child, this session is for the parent(s) only so we can talk candidly about your concerns and then schedule your child’s first session after that.

INSURANCE

Our office operates as an out-of-network provider. Your counselor can provide you all the necessary paperwork for you to file for insurance reimbursement. Generally speaking, most insurance companies will pay a percentage after you meet your deductible. Check with your insurance company for information about your specific benefits.

There are many reasons mental health professionals do not join insurance panels. The most important one is that filing with insurance companies usually requires a significant breach of client confidentiality. For example, to meet the requirements for in-network reimbursement, the counselor must submit a mental health diagnosis (DSM-5), and may also need to provide an ongoing progress report and modified treatment plans.  For children, a mental health diagnosis may follow them into adulthood and limit future opportunities for some military or government jobs. Most children, who come to therapy, are dealing with adjustments to life’s difficulties and not a mental health illness.

FEES

The fee for each 45-minute (play therapy) or 50-minute session is $80 to $265, depending upon the counselor’s level of education (Counseling Masters Intern, Masters or PhD), years of experience,  and the services provided. Payment can be made by cash, check, HSA/FSA, or credit card at the time of service.
